7/11/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Anthony upon arrival to the home. QMHP-C arrived to the home to conduct an individual session with Anthony. QMHP-C informed Anthony that the purpose of the session was to focus on Anthony demonstrating effective communication skills. 2. Behavior: Anthony appeared to be withdrawn during the second portion of the session as he was jumping off the couch to the floor with his younger sibling. 1.Intervention: QMHP-C processed with Anthony on the importance of maintaining a healthy living environment and the importance of good hygiene. QMHP-C discussed with Anthony that he places everyone in the home at risk of rodents and other insects in the home due to nastiness as well as risk with catching the virus when he does wear his mask at other people house when playing with his peers. QMHP-C processed with Anthony on coping skills and how to better adjust to stressful situations encountered. QMHP-C processed on limitations and compliance with household limitations. 2. Intervention: QMHP-C identified healthy outlets to express strong feelings and emotions and primarily for the developments of healthy bonding. QMHP-C reviewed compliance with authority both in the home and community. QMHP-C and Anthony engaged in an activity to practice using conflict management skills to provide cooling down period to reduce strong emotions. QMHP-C discussed frustrations within the family and effective communication to resolve issues. 1. Response: Anthony was receptive to QMHP-C's interventions and services that were provided to him by QMHP-C during the session. The mother reports that she struggles with Anthony's behavior at times as he never wants to participate in household limitations and follow set rules. Anthony show some frustration toward his mother and used positive communication after calming himself down. 2. Response: Anthony stated to QMHP-C that he does clean his room and takes care of his personal hygiene. Anthony stated to QMHP-C that he will double check his room the next time to ensure that things are not out of place.
